By completing, signing, and submitting this form, the Client confirms the accuracy of the provided information and authorizes the laboratory to proceed with the requested services. The Client acknowledges that the laboratory does not guarantee or provide any explicit or implied warranty. By signing, the Client verifies that the information supplied on this order form is correct and acknowledges that sample descriptions, information, or testing requests cannot be altered once laboratory work has commenced. The laboratory shall be released from any liability associated with commercial or legal consequences resulting from the sampling or analysis request information submitted and signed off by the Client.
